FEE
Individual Therapy Services: Sessions are 60 minutes in length. Each 60-minute individual session is $175.00.
EMDR Therapy Services: I offer 60-minute or 90-minute sessions. Each 90-minute session is:$190 per session.
Sliding scale- Based on availability and client need.
I offer 30-45-minute consultation sessions on a pro-rated basis which are appropriate for crises management, coordination of care, and collateral phone calls).
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
• You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
• Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
• If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
• Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/n CMSosurprises or call the California Department of Insurance at 1-800-927-4357.
PAYMENT
Payment can be made by credit card and is expected at the time services are rendered unless other arrangements have been made.
INSURANCES
Using insurance is really a personal choice and it’s important to know the benefits and potential drawbacks of using insurance. A mental health diagnosis will be required to get reimbursed for your therapy fees. Feel free to discuss the pros and cons of getting a mental health diagnosis with me if you are on the fence and need additional support making this decision.
I accept Blue Shield of CA for UC Berkeley Student Health Insurance Plan (SHIP). Out of Network Benefits. If I do not accept your insurance, I am considered “Out of Network”. If you wish to obtain reimbursement from your insurance company, I can provide you with a Superbill for services rendered. I do not contact or deal directly with out of network insurance. Contact your benefits administrator to inquire about out-of-network coverage for mental health services and ask the following questions before scheduling an appointment:
Do I have out of network mental health benefits, and do you reimburse me, if so what percentage is reimbursed?
What is the procedure for reimbursement?
What is my deductible and has it been met?
How much does my insurance plan cover for an out of network provider for procedure code (CPT code): 90791, 90837, or 90834?
Is a mental health diagnosis required for reimbursement?
